No your 5 hour course is another $50. And your pictured permit 7-10 days normally.

You will need to call and schedule the test, but yes, he can still take it.

Not sure but it's not expensive and please do it at your local DMV that way the money stays in the county instead of going to the state coffers

Youl need to bring proof of insurance, DL, the amount will depend on type of vehicle, weight, year etc., the supv name is scott, very nice gentlemen
