For almost a year, an employee at the 11101 Queens zip address of NYC Dep't of Health & Mental Hygiene repeatedly sends a vicious and false warning letter initiated by a neighbor. The neighbor works here (a nurse?) or has someone else here misusing taxpayer $$ to send over 110 copies, sometimes 3 a day. Rogue employee, clearly unsupervised facility. Wonder how they facilitate better health if they are preoccupied with harassment. Reported to their inspector general and to Commissioner Dr. Ashwin Vasan.

Wish I could give it 0 stars. The worst service ever. It took me 9 MONTHS to get a freaking birth certificate. The staff is unresponsive most of the time, and passive-aggressive when they respond. Awful service!

A 4yr wait for simple corrections, horrible customer service over the phone, driver license was taken away, money was never "Refunded" for services never received. This all smells REALLYYY "Fishy" here!🤔😠

This place makes the DMV look like the pinnacle of customer service. Absolutely awful. I am 5 months in to attempting to correct the hospital’s mistake on my son’s birth certificate, and documentation is constantly rejected for arbitrary reasons. I’ve wasted days of my life at this place. Avoid at all costs.
