Federal SSA says they're wrong and sent them a message supposedly. Attorney said they are "f@&%*$g R@%$#&d". Left on hold twice until they closed and was hung up on, the last time for 2 hours. Just found out I get Medicare three years late but nobody told me, still don't have a medical card or know what to do. Was awarded SSDI by the court over three years ago....still can't get them to turn it back on. It's a dumpster fire. According to the Lake City Office...it's all my fault. Fire them all!!! Straight up hung up on TWICE yesterday and its all recorded. They belong in jail. Fraud, Waste, and Abuse! Was left on hold for two hours before until the closed and was hung up on. That happened twice! Get a name and record your phone calls!

I was told 3 days ago that they will NOT accept appointments for a name change due to marriage. Therefore, walk-ins are the only acceptable way.
Make an appointment or go to the office. They can direct you on how or where to apply if needed and can tell you the proper paperwork you will need to get it started.

If you're under 18 your legal guardian can file for you. Over 18 you're set to do it on your own.
