Made an appointment for Tuesday at 10. Was told what to bring into the appointment including death certificate. Waited 3 weeks for the apt date. Drove to Beaufort. ARRIVED at office only to be told that they will make an apt for someone to CALL me in another few weeks for a "phone appointment". What a joke!!

You go online to the SSA.gov site and sign in or create account then print one. If you do not have a computer or internet you can visit the local office and they will print you one. If you go to local office, you just need your state photo id/license and your ssn card.

Literally down load, print, fill out and sign the SS-5 (Application for a Social Security Card). Provide an original (not a copy) ID -the DMV can make a Certified True Copy if you wish so you can keep your ID on you- and an original of the Name Change Document (if court ordered signed by Judge, if Marriage License the Bride/Groom Copy -which if recentlymarried is considered ID for SSA so just send Original Marriage Lic and SS5 in that case). Send to local SSA Office. They will send your original ID back after scanning in. Total time to having your new name if you do this approximately 3wks (mostly because of mail waiting times). :) check out SSA.GOV for more options on ID requirements. Have a great day/night. You need your birth certificate and discharge papers from a hospital. Something like a shot record with your full name on it sir. Sounds kinda crazy but they will not budge without it!
