If your family member is going to Broad Reach Healthcare in North Chatham, MA from Cape Cod Hospital... think of this, they are going from The Ritz Carlton to Motel 6. This Rehabilitation Center is an organizational mess. From the food services, attendant care, to amenities...it's shameful. As soon as you can get your family out of there, to a better facility (or home) the better. Your family member will recover quicker and with better care than at Broad Reach. It should be called Broad Reach Veterinary.

We worked with Broad Reach Hospice after my mother’s cancer had come back quickly and there was nothing they could do. We left the hospital at the end of October and luckily someone referred us to Broad Reach, Dr. Ward, the medical director came to our home and talked with my mother who was dying and not prepared for it. From there they created a hospice plan. In the next few weeks of her life she first got better, even though when we let the hospital they thought it could be days, but because of their excellent care her quality of life improved. The next few weeks the hospice team did everything they could for my mother. They responded to all her needs and concerns. They treated her with respect, dignity and love. They listened to her, gave her honest answers and information and walked her through the process of dying. She loved them and it eased her mind and body. I believe she died as peacefully and with as much acceptance as she could especially because she was not ready to die. They handled all the medical issues without it ever feeling like this was a medical situation. She got to live her days in her home with people who loved her. The hospice team also helped our family. The nurse and aid who came regularly were the kindest, most wonderful people. Full of positive energy and experience. They gave use the information, support and encouragement along the way. While they were there every step of the way, they also gave us the skills and empowered us to be involved with my mothers care during her last days. We didn’t feel like a spectator watching this happen to her, we felt part of it. When she died, I felt I had been through a life exchanging journey that I am forever grateful to Broad Reach for. If we had gone with my mothers original care plan that was suggested, it would have never been as meaningful, and peaceful. I am not sure we would of known there was another option. Dying is inevitable, Broad Reach made it as poignant and peaceful as can be. I can’t say enough about Broad Reach. Thank you.

We just enjoyed a very festive Thanksgiving feast with our 3 senior, loved ones at the Victorian, the assisted living facility associated with Broadreach, making it the perfect time to express how grateful we are to have the Victorian and Liberty Commons available to care for our family members. Navigating the aging process is a daunting task, and we have had nothing but positive experiences at both facilities. Every staff member has been professional, and personable, when addressing our concerns. And, they are never too busy to offer guidance or suggestions to ensure our loved ones comfort and safety. At both of these facilities, our family members are not only cared for, they are cared about.

My dad had taken yet another fall at home even though we had great care in place for him. He went to the hospital with nothing broken but I was told he would need “rehab” to get stronger. In my heart I new as a nurse he was failing and recovery was slim to none. I admitted him into Broad Reach “kicking and screaming” in my head and knowing the stay was gonna be as short as possible. Oh boy was this nurse aka daughter so wrong. The care was above and beyond anything I imagined. All, yes I said all the nurses, aides, and the staff that worked in the common areas were amazing. I had my demands like coffee, Gatorade, cookies and chips. Haha his staples, and they saw too it he got it. He was always clean, shaven and dressed and up for meals. Ok was everyday perfect? Of course not but it was a place where I could leave and go away for a few days and or go to work and not worry for sure. Ahhhhhh!!!! I didn’t want him to die there. I wanted him home. But in the end even for this nurse it was going to be too much. Tough call. I stayed with him by his side for his last seven days. I then realized we were home. We were with people that cared for him so I could just love him. If your love one gets a bed at broad reach and there’s no way they can be home—- consider your family lucky enough. This daughter feels lucky enough.

Thank you for your inquiry. Donations may be made in one of the following ways: To Liberty Commons’ resident council fund, which supports cultural programs for our long term care residents. To Broad Reach Hospice memorial fund, which underwrites individual patient and family special wishes at end of life when they do not have resources to pay for them and they are not covered services under their hospice benefit. To Broad Reach Employee Fund, which is used to bring special programming or other “extras” to our frontline caregivers. All checks may be sent to Broad Reach Healthcare at 390 Orleans Road, North Chatham, MA 02650 with the applicable program noted in the memo field. Again, thank you for your consideration.

what are the visiting hours

There are no set or limited visiting hours at Liberty Commons. Individual patient/resident choice is the primary guideline, and respect for roommates in shared rooms is also critical. Coordination with medical and rehab therapy appointments may also be a factor. Nursing staff will always be happy to offer guidance on an individual basis, too!
