My 86yr old mother is receiving wonderful care at Pine Creek skilled nursing facility. They have a great re-hab/exercise facility to get folks back on their feet and closer to independence. The Nursing staff are kind and patient, and the food is tasty. Thanks Pine Creek.

For being a SNIFF facility, this place was surprisingly clean and filled with friendly staff. I would recommend

Thank you to Sharnelle in Admissions! (I’m sorry if I spelt your name wrong) She’s been so helpful trying to get my mom placed. Looking forward to a brighter future for my mom and myself.

Questions & Answers

Who can i talk to about a problem?

Hello, There are a couple avenues for any concerns that you might have. You might ask who the PAR Room manager is for your family member. They should be checking on their assigned residents regularly. Another is the Social Services Department. If it is a clinical issue, the DON is a great resource to meet with. And of course you can talk to the administrator.

What is the P.T. rehab program like in this facility? My sister is only 44, and needs good P.T. Also, what has everyone's experience been in this facility?

This facility has a large therapy department, as Pine Creek carries a large case load of short term patients that are working towards getting back to their prior setting. We have a number of Physical Therapists (PTs), and they are supplemented with Physical Therapy Assistants (PTAs) that work under the PTs license.

Do they take tracheostomy patients.

Pine Creek care center does not take tracheotomy patients. Often time you would want to go to a Subacute facility for that. In the Sacramento area there are two SNFs with Subacute beds, Sacramento Post-Acute and the other is River Bend Nursing Care.
