Made me go home to get documents and it says they close at 5. I am here at 4:35 and they said that close that at 4:30. Two trips here ridiculous!!!! Not even friendly or helpful 😡

You can pay with your Debit or Credit card, but there is a charge of 2.75%.

Are vision tests for persons over 80 available in Lake Placid, fl? Also, do I have tomake n appointment or just walk in? Than you!

Just walk in and I believe they're going to give you a vision test they did with me just because my license is expired and I'm nowhere near 80 yet

I guess all forms of of IDs probably birth certificate social security card Bank information credit cards all of that will help you. I am not an attorney or an expert in the field but that will be my guest

Must get application form and have it filled out by your physician. Then take it to the tax office. Don't think it can be mailed.
