I made an appointment at this acceptance facility on September 23rd for a passport. I had a 1 year limited passport and was told by a third party passport agency that I needed to fill out the DS-11, I needed to do a sworn statement at an acceptance facility, and send my application in. I argued with the lady for about an hour and a half and she kept insisting I was wrong. Her exact words were “that form is ONLY for first time passport applications.” I eventually gave up and said I would just listen to her and filled out the DS-5504. 12 weeks after my passport application has been denied and to no surprise the letter from the Charleston office said I needed to reapply with a ds-11. When I was in the office I told the lady for my line of work it is vital that I have a passport... and I stressed how important it was that I did the application correctly and got it back as fast as possible. Please make sure your staff are fully up to date regarding passport form and policies. We should be able to trust their advice!
