1-888-242-4464--Call this number to be screened or call Flagler County Senior Services (a Division of the Health and Human Services Department) to speak with a case manager to ask questions about the program.
You must get in touch with the Department of Children and Family Services. Visit the state agency's website at myflfamilies.com.
Flagler County Human Services does not assist with co-pays for insured residents. The assistance they provide is directly to participating care providers who are willing to accept certain rates. They can only provide assistance for low or very low income applicants with referrals from a primary care physician (usually by means of a sliding scale clinic or the Flagler County Free Clinic) who do not have access to any insurance and would not qualify for Medicaid.

You might call Elder Source to see what services you are eligible for: 1-888-242-4464. Other welfare programs in Florida might include food stamps, for which you would contact the Department of Children and Families to apply. You can visit myflfamilies.com.
There are plenty of paid service locally but you can try the Disabled American Veterans on Florida Park Drive. 386-439-2122. I know they offer rides and other service to veterans. Thank you for your service. You will love Palm Coast!
This is not a Social Security office, despite it popping up when one searches for Social Security Office in Palm Coast (or Flagler County). The most local Social Security offices are located in St. Augustine and in Port Orange. This facility operates local community and social services such as the Adult Day Care Center, Senior services such as Meals on Wheels, Senior Case management; Human Services (temporary financial stability, indigent health care), and some housing services (homeownership, housing repair and rehabilitation).
