Got my daughter for School Checkup & Varicella Vaccination but ended.up giving my daughter an Incorrect 3In1 dtap Vaccine that she didn't need. Also her vaccination record was inpur incorrectly & Inaccurately. Vaccine Names & Dates were either incorrect or missed. This is absolutely medical neglience!
