My original birth certificate is on a microfilm and I misplaced it. I've called multiple times to California department of vital statistics and they advised me I have a state registration number (top right) and my record is able to be located in their system with that number and picked up in Sacromento through department of Health and vital statistics. Anyways.. you can go directly there and get the paperwork and fill it out.. then pick up the official record within 24 hours. 🙌 Great for traveling documents or driver's license! I told them I was going to fly in and was told to just come on down!

My review is solely based off one person. Tara was extremely short with me and disconnected my call after I asked her if I could explain my concern further. She said if I opened my mouth again she was hanging up and that I would be lucky if she even took a report or made note of it. No foul language was used and the only question I posed to her was “if I can’t explain my concern how can it be solved”. It was in regards to a 2 year ongoing interstate matter. She’s disgusting. After another 30-45 minute wait on hold I spoke with the most helpful gentleman who documented my situation, gave me helpful advice, and continued to ask me if I needed further assistance. If you need assistance it’s worth the wait to speak with someone more helpful.

Sent application and 26$ fee for birth certificate name change back in February, it is the end of May and I still have not received anything, some random lady called my number from U.S. Department of Health and Human Service Agency in March about if I wanted to get a COVID shot, so I'm pretty sure they received my application with the 26$ money order...

I read the reviews here and expected the worst when calling in, but surprisingly I had such a great experience! The rep who answered the phone named David went above and beyond to be incredibly helpful in locating my file and sending me any necessary documents. He even took the time out to walk me through each step to ensure I fill everything out correctly for next time, which I really appreciated! Wait time was about 8 minutes until I was connected. It is a bit of a pain amending anything because everything is so technical, but it makes sense because they’re official vital records. Thanks David for making everything so painless!
