I had great experience with this field office due to covid 19 restrictions they only allow limited in person appointment ,no walk in I had to make phone appointment then come in to submit the form in person the officer Rick was excellent so helpful, kind and super professional. I really value their hard work. Thank you so much for making my office visit experience so positive.

In early May 2021 my grandchild who is 18 didn't have his social security card it was lost for a few years. Ofcourse their office was close due to Corvid19, so you couldn't make an appointment go in person.. He went to Ukiah office with forms, copies of his birth certificate and ID. The security guard told him to slip it though the door. After a few weeks they sent a letter stating that they don't accept copies. Again he fill out another form , put in Original Birth Certificate and Original ID took it to Ukiah office in an envelope as before slip it though the door. He called a few weeks later and they said they lost his Birth Certificate! To call in few weeks. He called couple weeks later and they told him he put down wrong zip code and he had start over! They still didn't find his Birth Certificate! So now he has to pay again $35. to get another Birth Certificate and fill out another form but now he doesn't have his ID ,so they probably will not approve this time even though they have his only ID!! (why can't they change zip code??) He can't get a state ID or driver's permit without a social security card! Its so outrageous! Oh I didn't mention that he has a disability!

The Ukiah office is empty. Just a security guard with an attitude.
Contact the office directly. Each person receiving a SS payment must have their own bank account for direct deposit. For example if you have minor children receiving payments, they also need their own accounts, otherwise a live check will be mailed.
If you have a my social security account online, I believe you have access to your statements. If you're not sure I would go to the local office and ask for the statements that you need, also ask about the my social security account and they will help you set that up. Hope this helps you Alison.
