Not sure you can. Easiest way is go there, fill out form, $25 to cashier counter, take receipt and form back to vital statistics counter.
No. But they will give you all the information that you need to get them.
$25. Available same day if you get there before noon. If after 1 pm you pick up next day. Also, you pay up when you order.
