Helped us when no one else would. They cared for us and helped us navigate the government paperwork. I will forever be grateful for them.

The agency terminated my case worker and my case set idle for three months. Repeated calls to the only number I had went unanswered. No one picked up the slack. As a result I was left holding a cost of three months of assisted living. Follow up has proven to be useless. People would rather argue than accept responsibility.

He may qualify for LTC if the state determines that he’s in enough medical need and if he qualifies financially. He first needs to be assessed by phone and placed on a waiting list. The Alliance does not determine who gets off the waiting list. The more frail a person is and the more hardship a caregiver faces, the higher the chance they can get LTC.
Dr authorization get form and apply they will do interview and mail form to you if pre qualifying
