Getting a quote for monthly membership fees was like pulling teeth. Other gyms just show the price on their website but Forma wants you to submit a form with all your personal information before they'll tell you. Calling to find out the price wasn't much better. Sorry for the bad review but wasn't any reason for that to be so hard. Edit - Better response from Forma would've been "Sure we'll update our website to show the price of membership" so people don't have to write a bad review just to find out how much they'd be paying. It's crazy to even write all this.

I just had the best complimentary massage from Andrew! He is a great therapist and very informative. Gave me some tips regarding my shoulder issues. I really like the fact that this gym gives complimentary massages, never seen that before and much appreciate it.

love FORMA. plz find a water aerobics instructor for the WEDnesday and Friday AM classes! also, if you are tasteful enough to buy new flowering plants to beautify the front, do not forget to water them! Helen PS Love FORMA!

Best gym in town. Have been a member last 3 years and can’t say a bad thing. Nice equipment, clean, but staff is the best part of it. Torey, you the rock, brother!

I pay $90/mo. It depends on the plan you choose and when you sign up. My plan is from a year ago.
There's kids' supervised room while parents exercise. Older kids occasionally seen with a parent. There's family pool time Sundays from noon to 5 pm during summer.
