If you're 16 or 17 and want a tattoo or body piercing, or if you're under 18 and want an ear piercing of any type, you must have our consent form signed by your legal guardian and you must have the form notarized.

Pretty sure it's 16+. And if you're under 18 you have to get your paper signed by a parent or guardian and notarized. It's probably best just to call and confirm with them.
