Tried to get through to customer service 4 times, each time I was disconnected. This is not customer service. Very frustrating.

They say I owe more than my co-pay for a visit with a specialty doctor. All I did was talk to the guy. Reason makes absolutely no sense. Phone line has been busy all day. Finally reached them with a number not on my card. had to wait on hold for very long time to finally have them say they resubmitted it for review and it was done in error but wasted half my day.

Agreed. The absolute worst. Make me jump through hoops even though we have been with them FOR YEARS and I've been on the SAME MEDICATION. Every two months it's like an act of Congress to get medication because there are "other" which I have tried but my Docs office needs to send them that information....WHICH THEY ALREADY HAVE....once again for years. It's beyond frustrating and then I have to wait for weeks off my medication to get it all sent through. I can only imagine for folks that don't work from home and have the time or energy to fight for their medications. Appalling.

I changed insurance, and my coverage started July 1, a Saturday. July 4, a holiday, is Tuesday. Not only are the offices closed on Saturday and Sunday, but Monday, too. I got a letter dated June 27, 4 days before the first. It included a member number but no phone and no other useful information. This is now Monday. I am hoping that neither my wife nor I is sick today or tomorrow and that on Wednesday, when we need a prescription refilled, you will be so efficient that there will be a way for us to get the drug for the normal co-pay and not for the retail price. I have no problem with celebrating the holiday, but this is a health care company and needs to have coverage for non-business hours. We are starting off our coverage on a bad foot.

It should be on their website under contact us...
Depends what type of test and what type of coverage/plan you have. Call the # on your ID card.
Do mean out of network providers??
Is there any difference in premiums if services are provided by the OUT OF STATES providers?
Depends. Did the member have insurance through their employer or did they pay directly? If employer then you shoud contact their human resources department directly, if not, call the number at the back of the insurance card.
Depends on your plan. Call no. behind your id card.
