Avoid this location at all costs. Had to wake up at 445am to get in line here by 6am from Wimauma just to be told at 8am they they are only taking 7 families today. With a line of 60 people here, why are they even open?? You have a better chance winning the lottery than getting your kid an immunization shot here. This is the 2nd day in a row we came and was told the same thing. Someone fix this system they have going on here or just close the place down seriously. A waste of people's time, and looking at the other reviews, many feel the same way over the last few years. My kid still can't get his shots and is now missing 3 days of school.

No sense in even having an appointment. My appointment was for 3pm it's now 4:10 and I'm still waiting.

So the staff seems nice enough however the process here is beyond pathetic! Here at 6am waiting to open, get registered by 7:30, one of the first. Told to be back at 10:15 and was back on site at 10. Here it is 1:47 and i am still yet to have my daughter seen...... HORRIBLE

Nurses are just hanging out with the receptionist and making people wait hours past their appointment time. They want to get paid to gossip and waste peoples time

Pertussis is associate and combined with diphtheria, tetanus (lockjaw) and pertussis (whooping cough), formally known as Tdap. A prescription nor doctors note is needed for this vaccine. Please call 813-307-8077 to schedule an appointment.

Most likely no, unless by some policy. If a location is administering vaccinations, they should have access to Florida Shots where they can look up your vaccination history if needed. However, it would not hurt to have your card as a backup.

In order to confirm that our organizations has your immunization history on file, some additional /personal information is needed. Please contact our office at 813-307-8077. Please note: • If your Immunization records is on file, no appointment is needed. Clinic location is 8605 N. Mitchell Ave Tampa FL, 33604. Clinic Hours of operation is 7:30-4pm Monday-Friday. • If the Health Department does not have your immunization records on file, your option will be to contact the school board when you attended either kindergarden,7th-grade, or 12th grade: examples (Hillsborough, Pinellas, Polk, etc.) school board to request a copy that they may have on file. • If you are not able to come in personally because you are residing at a different state, an out of state county health department, primary care physician, or urgent care may requested on your behalf. This request however must be on an authorize consent form or company letterhead form from the location I have stated.

The parent will need to bring their photo ID in order to obtain vaccines or vaccination records. If it is a religious exemption the birth certificate is also needed. The Immunization Sulphur Springs Health Center address is 8605 Mitchell Ave. Tampa, FL 33604. This listing and the 8515 address is for Tuberculosis only. The buildings are separated by a street.

We do not give travel shots anymore. In Tampa, FL we refer those needing travel shots to Passport Health. Their phone number is (813) 969-3757. They should be able to assist you.
