My father arrived at Keystone unable to walk and talk, but staff changed that and he's so much better now. Thank you, Keystone, for the miraculous work that you do.

Highly un recommend ! My grandmother is on her deathbed. She was in a very good home for a few years. After 2 months of her being at keystone. She is now on her deathbed. DO NOT send your loved ones here under any circumstances.

Do NOT send loved ones here. The level of care is HORRIBLE. How anyone who works here is able to go home and lay their heads on a pillow is truly baffling. Imagine if others provided your loved ones the same level of care as you provide to others. Eldery neglect is the level of care to expect here! Your facility should be closed & 95% of you should NEVER be allowed to touch another patient!!!

An elderly friend was transferred to this facility from the hospital for short-term rehabilitation prior to entering an adult living facility, and his experience was horrible. Immediately after transfer, he was supposed to see an in-house doctor. That didn't happen until three days after his admittance. They screwed up his medications, and acted as though they barely knew he was admitted. He was there for two months as they waited for his long-term care to be approved, and during that time frame, he had several nurses complain about the management and the nursing staff was like a rotating door with nurses leaving and new ones coming consistently in that short time period. Also during his stay, the State health department apparently conducted an audit and two of the nurses told my friend they were under investigation and were given strict deadlines to come into compliance or face shutting down. There is so much more to tell, but in summary, this facility is extremely disorganized. Most of the staff is unhappy and unfriendly. They are very difficult to get a hold of by phone and in person. And don't bother leaving messages because they never return your calls or respond. I tend to give a lot of grace to those working in medical or senior care, because I understand it is a challenging field. But, this facility barely performs even the smallest level of care in my experience as an observer and advocate for my friend. I wouldn't recommend this place to anyone.

Call 911 and do a wellness check on the person that is in care. I had to do this to get the facilities attention. The more people do this will most likely result in police getting suspicious and start questioning them. Noone wants the police snooping around. They are ridiculous with this.
This depends on whether your doctor has privileges in the center where you are admitted. If he or she is not credentialed for your facility, then your case will be referred to another doctor who is on staff there.
Generally speaking, a skilled nursing facility is a clinical care provider of 24-hour licensed nursing. A SNF , as they are often referred to, is primarily engaged in providing services for residents who require medical or nursing care and/or therapy services for the rehabilitation of injured, disabled, or sick persons. Care typically includes: rehabilitation, intravenous therapy, post-surgical stabilization, pulmonary management, and wound care. Additionally, facilities provide assistance with administration of medications, and aid in a variety of daily living needs, including dressing, bathing, walking and eating.
Social services staff works with our residents and their families during times of transition. Our team provides information, addresses concerns and requests, and helps plan the discharge of each resident. Social Services can arrange professional services for any of the following: vision, dental, audiology (hearing), podiatry, psychosocial/psychiatric evaluation, creation of advance directives, dementia assessment, and individual therapy.
Physical, occupational, and speech therapies take place throughout the week. You will be evaluated after you arrive, then a personalized therapy plan will be created to best meet your needs. Wear tennis shoes and comfortable loose-fitting clothing.
Our activities and programs are created with residents in mind. We plan activities that suit the needs and capabilities of each resident. We find that this helps the healing process of our rehab patients. Activities include fitness programs, singing, trivia and games, religious meetings, musical performances and other entertainment. Families are encouraged to participate with residents. An activity calendar is provided so residents can stay informed on upcoming events like parties, movies, outings, ice cream socials, etc. As always, family and friends are invited and encouraged to attend.
Prepare an inventory list of items you re bringing with you. -Medical records -Copies of living will and power of attorney -Copy of Medicare/Medicaid or insurance card -Several days worth of comfortable clothes, sleepwear and toiletry items -Something from home that might bring you comfort like a favorite quilt, photos, reading material, etc.
