I am a retired Marine connected with the La Jolla VA. In my situation, I requested service at the naval medical center through my specialty care coordinator at the VA. Once I got to NMCSD I scheduled an appointment and consult for physical therapy through an internal specialty care manager (or PCM would work too). It's not simple, but it is great care and quick appointments once you start care at NMCSD

You must go in person to the records department and ask for a copy. They can not fax them to you. The records department is to the left in the same area as the pharmacy.

I guess I would just call the main number of the NRMC (balboa hospital) and take it from there
