Keeping some of my negative comments to myself, it's worth driving to the Salem location. 1 1/2 hr for a 2 minute transaction. And no, they aren't THAT busy.

In and out in less than an hour this location always has the best stuff very friendly very helpful and encouraging people sounds strange to say about a DMV but this is one office I don't mind going to because they make it painless

It cost $39.50 for the ID and you will need your birth certificate and proof of address. So some kind of mail like a phone bill or something.
Make sure you bring the insurance card for the vehicle you're going to use and make sure it's the card that hasn't expired yet. Make sure the cars headlights, tail lights, horn, brake lights and turn signals work, along with the car being clean. Park the car where the yellow test parking signs are located and don't back into the spot, just park like a normal person. It's $9 for the test and either $25 or $60 depending on your legal presence status. Also, when you come into the office, stand by the second pole and let them know you are checking in for a drive test.
Yes, as long as it hasn't expired yet and you have the renewal notice in hand. There's a code you have to use in order to finish the process.

You need to have an appointment. Go to ODOT DMV to find out about appointments and what documents you need to bring to the appointment. There’s about a 6 week wait for appointments.
